Attention parents of 6th graders...Your child will not be able to attend school without the Tdap immunization, or a letter from your physician, stating that you have an appointment scheduled to have the Tdap immunization. If you have not already provided proof of Tdap immunization, please fax proof of immunization, or your doctor's appointment letter, to 256-4209. Questions?
